The deeply discounted care and added benefits for members are based on a 12-month cycle of membership. However, we understand that things come up, people move etc. We will allow you to terminate your membership with 30 days’ written notice, but you will then forfeit the member discounts that were provided. If you choose to re-apply at a later date, you must pay the registration fee again.
If the value of the services you receive over the term of the membership exceeds the amount you paid in membership fees, you will be charged an amount equal to the difference. The value of the services is equal to AmericanHealth Mobile Medicine’s usual and customary fee-for-service charges (non-member prices).
If you have paid your membership fees but have not utilized the services, we will refund the unused portion of the fees on a prorated basis.
If you do not feel comfortable with a 12-month commitment to the doctor-patient relationship, then it is best that you not start a membership.
The primary function of a home health nurse is to follow the physician's plan of care, administering medically necessary services to treat, prevent, or manage the patient's condition. At each visit, the home health nurse will take the patient's vitals, track symptoms and other details of the patient's condition, and administer any necessary medications or treatments. Home health nurses may also keep a journal for each patient, recording the details of the visit to ensure continuity of care and communication between family, caregivers, and the patient's medical team.
